If the patient was having pain due to an orthopedic device, implant or graft / joint prosthesis the code is 996.77. See the Tabular List under 996.7 to see that pain is included. You would also code V43.65. However, this may be a case where you need to get clarification from the MD, since to me his diagnosis of "failed knee replacement" would indicate some type of mechanical complication, and not just pain.

I decided to call the surgeon's office to see what code they used. They used 996.46, so it was
considered a mechanical complication. |

Well....I just checked again and per the index: Failed / device, implant or graft - see Complication / mechanical. So based on the MD's diagnostic statement of "failed knee replacement" the index supports 996.46 also.
